Most nurses in German public hospitals are paid under the TVöD-PTarifvertrag für den öffentlichen Dienst (Pflege). It covers approximately 560,000 nurses in public hospitals, psychiatric facilities, geriatric care homes, and rehabilitation centres.

TVöD-P uses Entgeltgruppen (pay grades) P5 to P16, each divided into up to six Stufen (experience steps) that increase automatically with years of service. Your grade is set by your qualification, specialisation, and responsibility level — not negotiated individually.

How Überstunden (Overtime) Works for German Nurses

German nurses under TVöD-P have a standard working week of 39 hours. Hours beyond this are Überstunden. How these are compensated differs sharply from the UK NHS or US hospital systems.

Freizeitausgleich First

Under TVöD, the primary compensation for overtime is Freizeitausgleich — time off in lieu. If you work extra hours, your employer gives equivalent time back within a defined reference period. Cash compensation is secondary, not automatic.
